Case management is a consumer-focused strategy for linking seriously mentally ill (SMI) adults to necessary community support systems and services to improve their chances for recovery and/or successful community living, and to reduce the likelihood of individuals needing more intensive services.
Most case management services and activities occur on an outreach basis. The primary goal of case management services is to ensure effective service coordination; i.e., assessment, linkage, support and resource development, advocacy, social support, case planning, service arrangement, reassessment and follow-up, and monitoring, to assist with meeting the consumer’s human service needs.
For those who qualify, case management services are available in Lee, Russell, Chambers, and Tallapoosa counties.
