Every individual who receives services from the Integrea Community Mental Health System is treated in a manner consistent with the individual’s human and legal rights. Individuals have the right to:

  • Freedom from Discrimination
  • Privacy/Confidentiality
  • Access Information
  • Express Complaints an Grievances
  • Receive a Statement of Charges am Limitations on Services
  • Receive Information Regarding the Need for Parental or Guardian Consent for Treatment
  • Be Fully informed Concerning Services Provided
  • Information Concerning Research and Experimentation
  • Access and Privacy of Mail, Telephone Communications, and Visitors in Residential Programs
  • Access Courts and Attorney
  • Information about your Commitment Status
  • Human Respect and Dignity
  • Refuse Services
  • Be Informed of Accessing Rights Protection
  • Be Free of Punishment
  • Adequate Food and Shelter in Residential Programs
  • Access Medical Services in Residential Programs
  • Receive Appropriate Services Subsequent to Commitment
  • Informed Consent for the use of Specialized Equipment
  • Be Informed that the Center will Report any form of Abuse
  • Enforce Rights in a Court of Competent Jurisdiction or Appropriate Administrative Proceeding
  • Protection of Your Personal Liberty
  • Exercise the 5ame General Rights as Other Citizens of Alabama

I understand my rights at Integrea Community Mental Health System. My rights have been explained to me, and I have received a copy of the Center’s Rights form.

I understand that under certain circumstances, such as a safety, medical, psychiatric, or behavioral emergency, under a Court Order, or in response to known or suspected abuse or neglect\mistreatment or exploitation of children, elderly adults, or adults with disabilities, information may be released without my written consent.

I give my consent (or the treatment I am requesting {rom the Integrea Community Mental Health System. If it is my child or ward who will be receiving services. I hereby give consent (or that treatment.

I give consent for follow-up by the Center for up to one year after case closure.
