RESPONSIBILITIES AND CENTER RULES
Realizing that the freedom to exercise rights carries with it the need to accept rules and responsibilities, the following list of responsibilities and Center rules is expected of individuals receiving services:
To provide, to the best of your knowledge, accurate and complete information regarding your medical history, including present and past illnesses, medications (both prescription and non-prescription), hospitalizations, etc. To inform Center staff of any changes in your physical health or medications you take.
To be responsible for your actions should you refuse treatment or not follow recommendations of Center professionals.
To protect the confidentiality of other participants in treatment by not disclosing the names or any Information shared by other participants.
To attend scheduled appointments and activities, and to take any medications prescribed by the psychiatrist as directed.
To provide 24 hours’ notice in the event you will be unable to attend a scheduled appointment or activity.
To be considerate of the rights of others.
To be respectful of the property of others and of the Center. Stealing is prohibited.
To take an active part in planning, implementing, and following through with your treatment, including discharge planning, and payment of all applicable fees. With your consent, your family or significant other may also take an active part in your treatment.
To ask questions when you don’t understand treatment, instructions, etc.
To not bring or use alcoholic beverages, illegal drugs, or unprescribed medications on Center property. Individuals who attend Center programs under the influence of alcohol or other mood altering substances will be immediately assisted in securing safe transportation home. Law enforcement officials will be notified of individuals who leave a Center under the influence without safe transportation.
The use of food, beverages, and tobacco products in designated areas only.
To refrain from physical fighting or any behavior that is dangerous to self or others on Center property or during Center activities. Firearms and other weapons are prohibited.
To wear seatbelts at all times when transportation is provided by Center staff.
To cooperate with drug and/or alcohol screens when clinically indicated. No one will be dismissed from a Center on the basis of a single positive drug screen.
