RELEASE OF LIABILITY

The Integrea Community Mental Health System provides a broad range of treatment and supportive services to individuals and their family members. Services provided or sponsored by the Center may be arranged with outside organizations including community based activities.

I acknowledge that while the Integrea Community Mental Health Center and its staff make every reasonable effort to appropriately supervise treatment/program activities and promote safety, I understand that safety cannot be guaranteed and all risks cannot be anticipated or eliminated.

I agree to hold harmless and hereby release the Integrea Community Mental Health Center, its employees, its volunteers, and its Board of Directors, from liability for any injury or damages sustained while participating in treatment/program activities.

I understand that this Release will be for the duration of my treatment at the Integrea Community Mental Health System. This release sets forth the entire agreement between the parties with regard to the Center’s liability for my participation in treatment/program activities.
